To make sure you are ready to complete the application we recommend you have the documents* listed below. You can use your phone, a tablet, or computer to complete the application.
- Your Social Security Number or Business EIN/ITN
- Your state-issued ID or driver’s license
- Proof of income with a recent federal tax form or TANF/Families First statement
- Proof of residence (may include: utility bill, voter registration card, lease agreement, letter from landlord or homeowner)
